Transitional Care Workgroup

The Improving Healthcare Systems (IHS) program convened a multi-disciplinary workgroup to discuss Transitional Care (from hospital to home). The purpose of the meeting was to identify unanswered questions that are important to patients and other stakeholders when deciding to participate in programs of transitional care.
