Connie Hwang, MD, MPH is the Associate Chief Medical Officer at Twin Health, a global health care company focused on personalized, data-driven approaches to reverse diabetes and chronic metabolic disease. She builds high-impact partnerships with employers, health systems and health plans that leverage Twin Health's wearable sensors and digital twin technology to help patients with chronic metabolic conditions achieve normal clinical values and stop medications.
Prior to Twin Health, Hwang was Chief Medical Officer and Director of Clinical Innovation at the Alliance of Community Health Plans, an association that convenes nonprofit health plans spanning 35 states and covering over 22 million Americans. She collaborated with clinical leaders across the country to explore ways to accelerate health care transformation, value-based care, community partnerships and health equity.
Previously, Hwang was Vice President of Quality at Evolent Health, a population health management company, where she led improvement initiatives related to Medicare Advantage Star Ratings. She also served as the Vice President of the Measure Applications Partnership—a public-private collaboration convened by the National Quality Forum consisting of over 60 national organizations and nine federal agencies.
Hwang earned her MD from the Mount Sinai School of Medicine and her Master of Public Health from the Johns Hopkins Bloomberg School of Public Health.
Hwang was appointed as a member of the PCORI Board of Governors by the Comptroller General of the United States in September 2020.
