Project Summary

Our group, Survive to Thrive: Living Well with Stroke, was formed during Tier I and has continued to evolve into a dynamic, passionate, and productive group. Comprising stroke survivors, caregivers, rehabilitation health professionals, nurses, and academic researchers, we have continued to strengthen our partnerships, network, and outreach. During Tier II, we developed comparative effectiveness research questions that seek to address the fragmentation in care and support services stroke survivors and their caregivers experience following discharge from acute inpatient and post-acute rehabilitative care. For Tier III, we have designed a plan that transitions the group’s ideas into a research proposal application. In addition to our research development efforts, we will continue to engage with stakeholders and participate in face-to-face and virtual outreach and community awareness efforts to expand our reach and positively influence the lives of those affected by stroke.
