Transitional Care
Care transitions occur when patients move between different healthcare settings, such as from a hospital to home care or between different practitioners. Poorly executed transitions can harm patients and lead to unnecessary hospital visits.
PCORI has funded 30 transitional care projects. (As of July 2022)
Sixteen states (along with the District of Columbia) are sites for the transitional care projects.
PCORI has awarded a total of $132.5 million in funding for the transitional care projects.

Supporting Parents’ Mental Health from NICU to Home
A PCORI-funded study — comparing a program with and without peer support for parents leaving the neonatal intensive care unit (NICU) for home — has led to improved mental health screenings and the formation of a mental health taskforce at Children's National Hospital in Washington, DC.
